The prerequisites listed below must be submitted to the LBS office prior to enrollment in the course.

* What, when, and where to submit prerequisites *

Spring 2024 registration (begins October 16, 2023), email the following documents to the LBS office at [email protected] once the spring class schedule is published*.*

Fall 2024 registration (begins April 15, 2024), email the following documents to the LBS office at [email protected] once the fall class schedule is published.

🚫 Incomplete submissions will not be accepted/processed. 🚫 Do not submit images; all documents must be attachments in PDF format. ✅ E-mail confirmation will be sent upon approval. Allow 2-4 working days for processing.

✅ Send one email to [email protected] with the subject line “Clearance Paperwork” and the following information:

1. Proof of #’s 1, 2 and 3 (below) must be attached and in **PDF ****format; do not submit images.
2. In the body of the email, include the following:
   • First, last name
   • Student ID
   • The semester LBS 302 and/or KIN 425 will be taken
   • The section number of LBS 302 you would like to enroll in (i.e., LBS 302-02)
   • If #3 (BSR) was cleared through advising, you must indicate the advisor’s name and the date advising was completed.
     • BSR clearance by the advisor is only valid for LBS 302. Official BSR clearance for the TED program must be completed by submitting the evidence form found at https://www.csudh.edu/coe/info-students/basic-skill/)

1. Negative TB test

A TB test can be done at your doctor's office or for free at the Student Health Center on campus.

Contact the Student Health Center for test availability at (310) 243-3629.

Tests done within four years from the date class starts are acceptable. Clear chest X-ray reading for positive TB results valid within five years of class start.

Proof of negative TB skin test or clear chest X-ray must be an official form with the name of the medical facility, displaying your full name, and birthdate.

2. Certificate of Clearance +/- $180, 4-6 weeks processing time

🚫 Do not submit a copy of the Live Scan Request form (step A). 🚫 Do not submit the email or certificate attachment from the Commission. ✅ Do submit the printout from the CCTC website, saved as a PDF, with the document number, document title, issue date, and expiration date.

It's a two-step process to obtain the Certificate of Clearance:

• Step A: Live Scan (electronic fingerprinting) for a background check. 💵 $126 (price may vary)
• Step B: 💵 Online Application: $52.50. Allow for at least 4-6 weeks for processing.
