* How, where and when to submit prerequisites *

**🗓️ For fall 2024 registration, email the following documents to the LBS office at [email protected] ASAP.

• Include proof of #'s 1 and 2 (below) in one email, in PDF format
• Make sure to include your first name, last name, and student ID in the e-mail
• Indicate paperwork is for KIN 425

<aside> 🚫 Incomplete submissions will not be accepted/processed. E-mail confirmation will be sent upon approval.

1. Negative TB test

A TB test can be done at your doctor's office or for free at the Student Health Center on campus.

Contact the Student Health Center for test availability during COVID-19 at (310) 243-3629.

Tests done within 4 years from the date class starts are acceptable. Clear chest X-ray reading for positive TB results valid within 5 years of class start.

Proof of negative TB or clear chest X-ray must be official, displaying your full name, birthdate, and information of the medical facility.

2. Certificate of Clearance

It's a two-step process to obtain the Certificate of Clearance:

• Step A: Live Scan (electronic fingerprinting) for a background check. 💵$126 (price may vary)
• Step B: 💵Online Application: $52.50. Allow for at least 6-8 weeks for processing.

🚫 Do not submit a copy of the Live Scan form (step A). 🚫 Do not submit the email from the Commission. ✅ Do submit the printout from the CCTC website with the document number and expiration date.
