The prerequisites listed below must be submitted to the LBS office prior to enrollment in the course(s).
Fall 2024 registration (begins April 14, 2024), email the following documents to the LBS office at [email protected] once the fall class schedule is published.
Spring 2025 registration (begins October 14, 2024), email the following documents to the LBS office at [email protected] once the spring class schedule is published*.*

A TB test can be done at your doctor's office or for free at the Student Health Center on campus.

Tests done within four years from the date class starts are acceptable. Clear chest X-ray reading for positive TB results valid within five years of class start.
Proof of negative TB test or clear chest X-ray must be an official form with the medical facility’s name, showing your full name and birthdate.
🚫 Do not submit a copy of the Live Scan Request form (step A). 🚫 Do not submit the email or certificate attachment from the Commission. ✅ Do submit the printout from the CCTC website, saved as a PDF, with the document number, document title, issue date, and expiration date.
